Welcome to Southeast Polk Community School District Enrollment! Whether you are new to our district or a returning student, we are happy you chose to join the Rams! We engage all students in learning a challenging curriculum delivered through quality instruction. Southeast Polk uses an online registration (OLR) to register students. By completing and signing the Online Registration, you are affirming that the statements contained in this document are true.

CAUTION: Knowingly providing false information on this form will invalidate the application. Falsification of any information or document required for residency verification or the use of the address of another person without actually residing there may result in:

  • Revocation of student enrollment
  • Being held liable to reimburse the district for expenses incurred to educate this student
  • Civil action resulting from fraud, negligent misrepresentation, and negligence

Important Information

  • The online registration form must be completed by the student’s parent or legal guardian in order to enroll your student at Southeast Polk.
  • Proof of legal guardianship needs to be provided in order to enroll the student.
  • Please upload the following information when registering online- see list of required forms/documents below.
  • Having all required paperwork — proof of residency, proof of birthdate, health physical, and immunization records will allow us to approve your registration more quickly.
  • Students won’t be allowed to start school without the required documents.

Required Forms

  • PK: Immunization, Health Physical & Proof of Age
  • Kdg: Immunization, Health Physical, Proof of Age, Dental & Vision
  • 3rd Grade: Vision Screening
  • 7th Grade: New Immunization record
  • 9th Grade: Dental Screening
  • 12th Grade: New Immunization record
  • An existing family who has moved to a different address: New Proof of Residency (see Proof of Residency examples in tab below)
  • Proof of Residency/Address Verification
  • Proof of age/birthdate
  • Health Forms

All new students entering the district must provide the proper documentation to establish residency to enroll. All documents must state the student’s physical address, and residency at that address is required. A post office box is NOT an acceptable address to determine residency. If you are new to our district, please be prepared to upload proof of residency showing your name and district address in any of the following forms. Failure to do so will delay your student’s enrollment at Southeast Polk.

NOTE: If an existing SEP family changes addresses within the district, a new proof of residency is required.

The only district-approved documents that are accepted for proof of residency include:

  • Official Homeowner’s Purchase Agreement (signed by you and the seller)
  • Current Official Lease Agreement (signed by you and your landlord with the company’s logo/letterhead)
  • Current water or electric bill mailed to you at your residence

Proof of age is required for all new students to the Southeast Polk Community School District. In Iowa, a child must be five years old on or before September 15 of the year he or she starts school to enroll in kindergarten. Proof of age is required prior to the start of school. For students in grades one through 12, proof is also required before your child can start attending school.

Several document options are acceptable:

  • A passport
  • Adoption papers
  • Immunization Record
  • A certificate obtained from the county where the child was born.

  • Certificate of Immunization. Your child’s physician can provide you with a copy of all immunizations given. A photocopy of the certificate of immunization is acceptable.
  • Vision Form. Your child’s physician can complete this.
  • Dental Form. Your child’s dentist can complete this.
  • Physical Exam. This can be completed by your child’s physician.

If custody is by other means than divorce, the Guardianship and Custodial papers must have an assignment of tuition. This is handled through the courts, so please contact your attorney for assistance.

Existing Family Registration Information

  • If you’re an existing SEP family living in the district and adding a preschooler or kindergartner, you need to register ALL of the students in your family at the same time.
  • Review and upload the Grade-Specific Required Forms & Information.
  • You must register all school-attending students annually.

NEW SEP Students/Families

Our Youngest Learners

  • Four-year-old preschool is available to all students of families living in the Southeast Polk District.
  • Four-year-old preschool is free to all students through the Statewide Voluntary Preschool Grant.
  • We are not currently accepting registrations for 3-year-old students due to increased enrollment.
  • Children must be 4 years old on or before September 15, 2025, to be eligible for preschool in the 2025-2026 school year.
  • We will do our best to honor home building placements for all preschool students. We give higher priority to students registered with the required paperwork prior to May 31, 2025.
  • Review the list of required documentation (see near top of this page) and upload them online. If you don’t have all the required documentation, submit your registration and forward the documents as soon as possible to district_registrar@southeastpolk.org.
  • Students won’t be allowed to start school without these required documents.
  • Please register/update ALL students in your family when registering your preschooler.
  • You will receive an email notification in early summer confirming your preschooler’s enrollment and notification of their building during the summer.

Wondering which elementary school your child will attend at Southeast Polk Community School District for the current school year?

Open Enrollment

Open Enrollment is the process by which parents/guardians residing in an Iowa district may enroll their children into another Iowa school district under the terms and conditions of Iowa Code. The open enrollment application deadline for the 2025-2026 school year is March 1, 2025, for grades 1-12 and September 1, 2025, for kindergarten.

Please follow the links for board policies 501-14 and 501-15 regarding open enrollment.

The receiving district may deny an open enrollment request if one of the following applies:

  • Insufficient classroom space.
  • Appropriate special education program is not available.
  • Student is under suspension or expulsion.
