Southeast Polk uses an online registration (OLR) to register students. We ask that you complete the OLR as soon as possible to allow our staff to complete the placement process in a timely manner.  

Please register/update ALL students in your family when registering your kindergartner.

New Families

If you are a NEW family to the Southeast Polk School District please click this link to begin registration. New Family Student Registration

If you are a NEW family in the district, you are required to show proof of birthdate and address verification.  Please upload these documents during your online registration process. These documents must be uploaded before you click submit.  Having all required paperwork — proof of residency, proof of birthdate, health physical, and immunization records will allow us to approve your registration more quickly. Students won’t be allowed to start school without these required documents.

Existing Families

  • If you’re an existing SEP family living in the district and adding a preschooler or kindergartner, you need to register all of the students in your family at the same time by clicking this link: Existing Family Students Registration

Documents needed to complete the registration process

  • Proof of residency. All new students entering the district must provide the proper documentation to establish residency in order to enroll. All documents provided must state the student’s physical address, and residency at that address is required. A post office box is NOT an acceptable address for the purpose of determining residency. One of the following documents must be provided:
    • Official Homeowner’s Purchase Agreement
    • Official Lease Agreement
    • Current water or electric bill mailed to you at your residence
  • Proof of birth date. Children must be five (5) years old by September 15, 2024, to qualify for kindergarten during the 2024-2025 school year. Several options are acceptable:
    • immunization record
    • passport
    • adoption papers
    • county certificate
    • state certificate
  • Health Forms.
    • Certificate of Immunization. Your child’s physician can provide you with a copy of all immunizations given. A photocopy of the certificate of immunization is acceptable.
    • Vision Form. This can be completed by your child’s physician. 
    • Dental Form. This can be completed by your child’s dentist. 
    • Physical Exam. This can be completed by your child’s physician. 

​NOTE: Falsification of any information or document required for residency verification or the use of the address of another person without actually residing there may result in:

  • Revocation of student enrollment
  • Being held liable to reimburse the district for expenses incurred to educate this student
  • Civil action resulting from fraud, negligent misrepresentation, and negligence

Kindergarten FAQ

I’m having trouble logging into the Parent Portal. What should I do? 

If you have forgotten your password and/or can’t get into your Parent Portal, please complete this automated form for assistance.

Will there be Kindergarten Information Sessions (previously called Kindergarten Round-up) this year? 

A: Yes, we are in the process of organizing the dates for Kindergarten Information Sessions. Typically, they are held in March/April. Look for information coming soon. 

I don’t have all the required documentation. Can I still submit my child’s registration? 

A: We would prefer that you submit all documentation at the time of registration to help ensure critical forms are all in place before school starts, however, if you are missing a document, you may submit your registration and then send your documents as soon as possible to
