When registering, ensure that you choose the correct year from the drop-down options:

  • 2023-2024 for this year or
  • 2024-2025 for next fall.

By completing and signing the Online Registration, you are affirming that the statements contained in the application are true.  CAUTION:  Knowingly providing false information on this form will invalidate the application.

Online Registration Required

Southeast Polk uses an online registration (OLR) to register students. We ask that you complete OLR during the month of February, which will allow our staff to complete the placement process in a timely manner.  

Please register/update ALL students in your family when registering your preschooler.

We will do our best to honor home building placements for all PK students. We give higher priority to students registered with the required paperwork prior to May 31, 2024.

Please note: Four-year-old preschool is available to all students of families living in the Southeast Polk District.  Four-year-old preschool is free to all students through the Statewide Voluntary Preschool Grant. We are not currently accepting registrations for 3-year-old students due to increased enrollment. Children must be 4 years old on or before September 15, 2024, to be eligible for preschool in the 2024-2025 school year. 

Documents needed to complete the registration process

  • Proof of residency. All new students entering the district must provide the proper documentation to establish residency in order to enroll. All documents provided must state the student’s physical address, and residency at that address is required. A post office box is NOT an acceptable address for the purpose of determining residency. One of the following documents must be provided:
    • Official Homeowner’s Purchase Agreement
    • Official Lease Agreement
    • Current water or electric bill mailed to you at your residence
  • Proof of birth date. Children must be four (4) years old by September 15, 2024, to qualify for preschool during the 2024-2025 school year. Several options are acceptable:
    • immunization record
    • passport
    • adoption papers
    • county certificate
    • state certificate
  • Certificate of Immunization. Your child’s physician can provide you with a copy of all immunizations given. A photocopy of the certificate of immunization is acceptable. Certificate of Immunization
  • Physical Exam. This can be completed by your child’s physician.  PK 6 Physical Form

​NOTE: Falsification of any information or document required for residency verification or the use of the address of another person without actually residing there may result in:

  • Revocation of student enrollment
  • Being held liable to reimburse the district for expenses incurred to educate this student
  • Civil action resulting from fraud, negligent misrepresentation, and negligence

New Families

If you are a NEW family to the Southeast Polk School District please click this link to begin registration. New Family Student Registration

If you are a NEW family in the district, you are required to show proof of birthdate and address verification.  Please upload these documents during your online registration process. These documents must be uploaded before you click submit.  Having all required paperwork — proof of residency, proof of birthdate, health physical, and immunization records will allow us to approve your registration more quickly. Students won’t be allowed to start school without these required documents.

Existing Families

Preschool FAQ

When will I know if my child is enrolled in preschool? 

A: There are a couple of steps to our enrollment process. First, we need to have all required documents: Proof of Residency, Immunization Records, Health Physical.  When received, your registration will be approved. The next step is for the Early Childhood Coordinator to enroll your student in the program. This is usually completed in early summer. At that time, you will receive an email notification. 

When will I know which building my child will attend? 

A: You will be notified via email in the summer

When will my child know who their teacher will be? 

A: Typically, all teachers are assigned late summer/early August. 

Can my preschool child go to the same building as their older sibling? 

A: We do try to place children in the same building as their siblings. 

I’m having trouble logging into the Parent Portal. What should I do?

I don’t have all the required documentation. Can I still submit my child’s registration? 

A: We would prefer that you submit all documentation at the time of registration to help ensure critical forms are all -in place before school starts, however, if you are missing a document, you may submit your registration and then send your documents as soon as possible to
